All questions about immunization requirements should be addressed to the Health Center Immunization Office at 301-314-8114 or send a message through the portal at www.myuhc.umd.edu.
The Immunization Form must be completed in English. This form must be submitted to the University Health Center before orientation. If you are in need of any additional immunizations, such as the Quantiferon Gold or T-spot, you will have an opportunity to make an appointment with the Health Center in September to receive these immunizations.
Step 1: Complete Personal Information
Complete the personal information in the section on page one of the form (your name, UID number, date of birth, etc.).
Step 2: COVID-19 Immunizations - Section A
Required for all students.
Step 3: Immunization for Measles, Mumps and Rubella (MMR) – Section B
The University of Maryland at College Park requires ALL incoming students to provide completed proof of immunization dates for Measles, Mumps and Rubella (MMR, 2 doses), and Tdap/TD within the past 10 years. A copy of a lab titer for Measles, Mumps and Rubella will be accepted in lieu of the immunization dates.
Step 4: Meningococcal (Meningitis) – Section C & D
Required for undergraduates and suggested for graduate students. A waiver opposing the vaccine (Section C) can be signed if the vaccine is not desired.
Step 5: Tuberculosis (TB) Testing Questionnaire – Section E
Every student must complete the Tuberculosis Risk Screening online at www.myuhc.umd.edu. If "yes" responses were provided on the Tuberculosis Risk Screening, then additional information should be provided in Section D. The Quantiferon Gold or T-spot must be done in the US within the past year.
Step 6: Optional Recommended Immunizations – Section F
Other recommended immunizations for all incoming students are: Chicken Pox (Varivax; 2 doses); Hepatitis A (2 doses); Hepatitis B or Twinrix (3 doses); HPV (3 doses); Influenza (yearly). They are not required for students at the University of Maryland. These immunizations can be acquired through the University Health Center, if desired, at a later date and by appointment.
Step 7: Optional Gender Identity - Section G
This section is optional for students that want to more specifically list their gender identity.
Step 8: Submission of Completed Documents
Immunization Forms must be submitted to the University Health Center before orientation. Forms can be uploaded to myUHC accessible through the University Health Center. You will be prompted for a Directory ID and Password, then you will enter your UID number. Go to forms, select Immunizations, and enter all the appropriate requested immunization dates found on your records. Click on Add Immunization Record - where you will scan your document to the record. Then submit final.
